By submitting this form you understand that we do not provide any water, electricity, etc. Your food truck must self-sustainable. I understand that this is an application and not a verification of approval for Acoustics for Autism on March 1, 2026. I further understand that I must be current with my State of Ohio licenses through the Health Department and with The City of Maumee Fire Department, which are usually up for renewal on or around March 1 each year.


By submitting this form, you understand this is NOT an acceptance of your application. You agree that Project iAm will determine the OUTDOOR location of your area and that a $100 non-refundable fee will be required with a signed contract, before confirmation of your space.  You further agree that you have read the stipulations listed above including “no materials provided” by Project iAm for your vendor space.  
 
As a 501(c)(3) nonprofit organization, we aim to create a welcoming and inclusive environment focused on supporting our mission. To align with this goal, we kindly ask that all vendor booths focus on crafts, handmade goods, and similar items. Unfortunately, we are unable to accommodate vendors with political or non ASD advocacy-related themes at this time within our permitted area.
